To be considered, you must have a diagnosed condition that hasn't improved with standard treatments prescribed by a doctor

What happens next

  1. Fill in the form

    Your pharmacy, your name and an email address — that's all we need to start.

  2. Finish in the Clinic app

    You'll be taken to the Curaleaf Clinic web app to create an account and add your medical history.

  3. A specialist reviews it

    You may be asked about medication you've already tried, or for your Summary of Care Record.

    You'll finish the rest of your details in the Curaleaf Clinic web app.
